The finance department has received the Government Finance Officers’ Association Certificate of Achievement for Excellence in Financial Reporting for the City’s comprehensive annual financial report.
Health Coverage 1095-C Tax Notification
Under current law, the City is not required to provide a copy of your IRS Form 1095-C, Employer-Provided Health Insurance Offer and Coverage. However, you still have the option to request a copy of your IRS Form 1095-C. If you have any questions or would like a copy, please contact Payroll at or 616-530-7225.
